- For current students only. Please use this form if you wish to continue your studies at BIA and your I-2o is expiring. This form may be used by students who aren’t on an F-1 visa and still want to continue their studies.
- For new students only.
- This is required by any student who has a financial sponsor. The name of the bank account holder must match the signature/name of the sponsor on this form.

Leave of Absence and Vacation Request Form
- Do you want to request a Leave of Absence or Vacation? Please submit this request form.
- Do you need to take a break from your studies due to a documented medical condition? Please submit this request form.
- Did you move recently? All students are required by immigration regulations to report this address to SEVIS within 10 days of moving. Please let us know by using this form.
Probation Extension Appeal Request Form
- Students that are currently on an Academic Probation status can use this form to request a Probation extension if they have failed to clear their status.
Letter of Acknowledgement Form
